Revision of Application Forms and Indemnification Criteria
We sincerely apologize to the general public for the tremendous 
inconvenience and anxiety that has arisen on account of the accident at 
Fukushima Daiichi Nuclear Power Station and Fukushima Daini Nuclear Power 
Station.

During the processing of permanent indemnifications for damages caused by 
the accident to concerned persons, we have considered implementing 
improvements to the indemnification procedures in response to comments and 
suggestions received from concerned persons.

As a part of our specific efforts, we have decided to implement the 
"Improved application form for individuals", "Indemnification revision for 
mental damage caused by evacuation etc." and "Estimated payment for 
concerned corporations and sole proprietors in financial distress". 

We have been making improvements such as the distribution of "Easy Claim 
Instructions" as well as providing assistance in filling out the 
application forms. In addition, we will continue to do our best to ensure 
that the process is as convenient as possible for all applicants.

1.Improved application form for individuals
  Improvements have been made to the application form for individuals for 
  fixed damages from September 1 to November 30, the 2nd period for 
  permanent indemnification from the perspective of reducing the amount of 
  pages and easy-to-fill-in instructions.

■Reducing the amount 
 ·By reviewing the application forms, we have reduced the amount of items 
  below.
  Number of different booklets (Before:13→After:9)
  Fill-in items in the application form (Before:2,115→After: 1,005)
  Number of pages (Before: 60→After:34)
 ·For individuals who have already reached an agreement on the initial 
  permanent indemnification, we have changed the procedures by printing 
  the given information on the application form beforehand.
■Easy-to-fill-in instructions
 ·A new booklet titled "Please read while filling in the application" was 
  issued in order to facilitate understanding of the overall flow of the 
  procedures and items for damage.
 ·For visual consideration, the layout has been changed by using more 
  colors.
 ·The sequence of the items in the application form was reorganized to be 
  consistent with the actual flow such as placing the "Total Amount" line 
  at the end of the application etc.

The application forms will be sent from December 2 and we will accept 
applications from December 5. As for the individual who has not yet filed 
an application for the initial time period (March 11 to August 31), it is 
possible to use the improved application form and send it to us at once. 
The information for the application form of concerned corporations and 
sole proprietors will be announced later.

2.Indemnification revisions for mental damage caused by evacuation etc.
  The burden placed on concerned people is expected to be significant 
  especially in the first year from the accident. Therefore, factoring in 
  the concept of the interim guideline, indemnification for "mental damage 
  caused by evacuation etc." will be revised as follows.

As for the living expense increase caused by the evacuation, applicable goods to be indemnified at actual cost are listed in the application form. (Q&A section in the instruction book) (The same content can be viewed at our website*) *Our website (Frequently asked questions regarding application for permanent indemnification) http://www.tepco.co.jp/comp/faq/index-j.html 3.Estimated payment for concerned corporations and sole proprietors in financial distress As for the corporations and sole proprietors who have already received payment of initial the permanent indemnification for lost earnings but are still in financial distress, we will make estimated payments for the second permanent indemnification upon request. Settlement for this estimated payment will be carried out with the permanent indemnification payment. [Summary of estimated payment] ·Eligible person: Corporations and sole proprietors with offices, etc within the designated evacuation area* ·Period of payment: Lost profit regarding the second permanent indemnification (Sep.1 to Nov. 30, 2011) ·Estimation method: Amount equivalent to 1/2 of agreed amount (monthly average) regarding lost profit at the time of the first permanent indemnification for 3 months will be paid. (Examination expense and additional cost will be excluded) (Ex.)
