Receipt of Instruction from NISA regarding report on investigation of cause and development of preventive measures regarding exposure exceeding dose limit to radiation dose engaged person at Fukushima Daiichi Nuclear Power Station
On April 27th, 2011, we received an instruction of "Investigation of cause
and development of preventive measures regarding exposure exceeding dose
limit to radiation dose engaged person at Fukushima Daiichi Nuclear Power
Station(*)" from Nuclear and Industrial Safety Agency (NISA),
Ministry of Economy, Trade and Industry.
(previously announced on April 27th, 2011)

Since then, we have implemented the investigation of cause and development
of preventive measures, based on the NISA's instruction. Today, we
reported the result of the investigation and the preventive measures to
NISA (partly amended on May 11th, 2011).

On May 25th, 2011, we received an instruction of "Evaluation results
regarding radiation control at Fukushima Daiichi Nuclear Power Station and
Fukushima Daini Nuclear Power Station (*)" from Nuclear and Industrial
Safety Agency (NISA), Ministry of Economy, Trade and Industry. Based on
these instructions, we will make a prompt and appropriate response.

On April 27th, NISA received the report of dose exposure to the person is
exceeding the dose limit set in the Regulation of Provision Concerning the
Installment, Operation, etc. of Practical Generation Reactors based on
Regulation Concerning the installment, Operation etc. of Practical
Generation Reactors (The Ministry of International Trade and Industry
Order No.187 of 2001, Dose Regulation) from TEPCO.
In response to this, Nuclear and Industrial Safety Agency (NISA) instruct
TEPCO "Investigation of the causes and development of preventive measures
regarding radiation exposure exceeding the designated dose limit to those
persons at Fukushima Daiichi Nuclear Power Station (Instructions) (No.4
April 27th, 2011 dated April 27th, 2011)" In this respect, TEPCO reported
NISA the result of "Report on investigation of cause and development of
preventive measures regarding exposure exceeding dose limit to radiation
dose engaged person at Fukushima Daiichi Nuclear Power Station" on May 2nd
and May 11th, 2011. NISA performed an evaluation of the report.

NISA acknowledged case following (1) to (3) in conflict with Regulation
Concerning the installment, Operation etc. of Practical Generation
Reactors (The Ministry of International Trade and Industry Order No.77 of
1978, Reactor Regulation). We regret that there was exposure exceeding
dose limit. For the following reason, we strictly warn TEPCO.

(1)The density of radioactive materials was exceed the statutory limit
   (0.001 Bq/cm3, iodine) at the Main Anti-Earthquake building at
   Fukushima Daiichi Nuclear Power Station, nevertheless TEPCO do not
   require wearing protection equipment for prevention of dose.

(2)Two female employees exceed the limitation level (5 mSv/ 3 months),
   furthermore, five female employees who do not appoint and resister as
   radiation workers were assigned for emergency work at controlled area.
   These issues conflict with reactor regulation and two of them exceed
   the limitation dose level (1mSv/year).

(3)TEPCO did not record the dose from March 14th to April 21st, 2011, 
   regardless the dose outside of building in Fukushima Daini Nuclear
   Power Station were exceeded the statutory limit at controlled area
   (1.3 mSv/ 3 months).

NISA also request TEPCO in view point of appropriate radiation control, to
improve the labor security of the employee, health care and improvement of
living for workers who appoint and resister as radiation workers. And to
comply safety regulation at Fukushima Daiichi Nuclear Power Station and
Fukushima Daini Nuclear Power Station, NISA request to preventive measures
as below;

1.Secure an enhanced structure for increase personnel for measurement of
radioactive for prior measurement and management radiation dose at site.

2.Secure dose meters for all employees to manage their radiation dose.
Until TEPCO acquire sufficient numbers of dose meters, under the condition
that TEPCO confirmed in advance the radiation dose is same in the
controlled area a representative can only carry it.

3.TEPCO should measure dose of employees who have not measured yet. All
the staffs are periodically measured once in a month (normally once in 3
month) for internal exposure regulated by reactor site safety regulation
at Fukushima Daiichi Nuclear Power Station and Fukushima Daini Nuclear
Power Station.

4.Implementation of health checkup following internal rule for appointment
/ registration for radiation workers in a rapid manner.

5.Ensure exposure level for radiation workers, restore the exposure level
system and ensure a steady implementation of record to radiation workers
resister center at Radiation Effects Association (REA). 

6.Reinvestigation for the numbers of female radiation workers which
reported to NISA on May 11th, 2011 was unreasonable on method for
investigation. TEPCO should make actions in order to take preventive steps.

7.In case there is event that conflict with the law, TEPCO should report
to NISA promptly.
