Improvement Action Report on Unauthorized ID Card Use and Partial Loss of Function of Nuclear Material Protection Equipment at Kashiwazaki-Kariwa Nuclear Power Station

TEPCO would once again like to sincerely apologize for the great concern and suspicion that it has caused amongst regional residents, and society as a whole, as a result of incidents involving partial incompletion of safety measure renovations, the partial loss of function of nuclear material protection equipment and unauthorized ID card use at the Kashiwazaki-Kariwa Nuclear Power Station.

TEPCO has received notice that additional nuclear regulatory inspections will be implemented regarding the unauthorized ID card use and the partial loss of function of nuclear material protection equipment. TEPCO has also received instructions to identify the direct and fundamental causes of this series of incidents pertaining to nuclear material protection at the aforementioned power station, and also signs of degraded safety culture and nuclear security culture (including third-party assessments), and report on improvement action plans formulated in light of this investigation by September 23, 2021.

TEPCO has conducted reviews in accordance with the instructions from the Nuclear Regulation Authority (hereinafter "NRA"). Results of root cause analysis regarding the unauthorized ID card use and the partial loss of function of nuclear material protection equipment at the subject power station as well as improvement action plans were summarized, and this was combined with the verification report from the third-party Independent Review Committee, comprised of members from outside the company, and were reported to the NRA today (September 22).

TEPCO will steadily implement improvement action to enhance power station safety and work quality as we utilize this opportunity to remember our regrets and the lessons learned from the Fukushima Daiichi Nuclear Power Station Accident and reincarnate our power stations. We will also respond sincerely to the additional regulatory inspection to be implemented.
